Job DescriptionFull‑Time LPN – Home Health $1,080/week | Mileage Reimbursement&nbsp; $1,000 Sign‑On Bonus (after 90 days)&nbsp; IRA + Health Insurance | Lawrence • Haverhill • Methuen&nbsp; Every Other Weekend Required Essex Home Health is hiring a Full‑Time LPN to provide home‑health visits for approximately 40 patients per week, focusing on hypertension and diabetic care. Key Duties 
<ul>
<li>Complete home‑health nursing visits (40 patients weekly) </li>
<li>Provide hypertension and diabetic patient care </li>
<li>Perform assessments, vitals, medication teaching </li>
<li>Document visits accurately and timely </li>
<li>Communicate patient changes to RN Case Manager </li>
<li>Work every other weekend </li>
<li>Travel between Lawrence, Haverhill, and Methuen (mileage reimbursement) </li>
</ul>
Requirements 
<ul>
<li>Active LPN license in Massachusetts </li>
<li>Home‑health experience preferred </li>
<li>Strong communication and patient‑education skills </li>
<li>Reliable, organized, and able to work independently </li>
