Job Requirements\n
- \n
- Active RN license and current nursing practice experience
- At least 1 year of nursing experience required
- Home health nursing experience strongly preferred
- OASIS experience required
- Strong knowledge of home care documentation, care coordination, and case management
- Ability to work independently while maintaining excellent communication with the interdisciplinary care team
- Experience with skilled nursing procedures such as IV maintenance, wound VAC, and complex wound care preferred
- Comfortable using electronic health records (EHR) and agency documentation systems

Responsibilities\n
- \n
- Assess patient and caregiver needs using the nursing process to develop effective home health care plans
- Serve as case manager for assigned patients and manage caseload productivity
- Complete accurate and timely OASIS documentation in accordance with regulations and agency standards
- Implement nursing interventions, procedures, and patient education to support safe care at home
- Perform and monitor skilled nursing visits, including wound care, IV-related care, and other home health services as needed
- Coordinate with physicians, therapists, social workers, vendors, and other disciplines to support patient needs
- Educate patients and caregivers on medications, treatments, discharge instructions, and reportable changes in condition
- Maintain thorough documentation and follow agency processes for visit completion, follow-up notes, and required forms
- Support safe transitions, discharges, and transfers while advocating for quality patient-centered care
