Summary: A registered nurse administers skilled nursing care to patients on an intermittent or daily basis in the patient's place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.

Qualifications:

Graduate from an approved school of professional nursing and currently licensed in the state(s) in which practicing is required.

Two (2) years of nursing experience preferred.

Ability to exercise initiative and independent judgment.

Must have a valid driver’s license

Must be able to pass CORI
-
Must be physically able to perform the duties of the position, including lifting unspecified weight, bending and stooping to prepare equipment for the client, or managing the client's environment
-
Transferring, turning, and positioning clients and assisting with ambulation
-
Must exhibit mature, responsible behavior and understand the need for patient confidentiality
-
Familiarity with basic nutrition and personal hygiene standards
-
Proficiency in English
-
Being bilingual is a plus

Caring and compassionate personality
-
Current CPR certificate
-
First Aid training
-
Able to work for extended periods of time while standing, driving, and being involved in physical activity.
Responsibilities:
-
Provide services in accordance with the patient's plan of care.
-
Make the initial evaluation visit and regularly reevaluate the patient's nursing needs.
-
Initiate the plan of care and necessary revisions.
-
Provide services requiring specialized nursing skills
-
Initiate appropriate preventive and rehabilitative nursing procedures.
-
Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner, as per Agency policy

Coordinate services
-
Refer to Physical Therapist, Speech and Language Pathologist, Occupational Therapist, and Medical Social Worker for those patients requiring their specialized skills;

Inform appropriate personnel of changes in the condition and needs of the patient
-
Conduct Patient Care Conferences on patients assigned to your care.

Counsels with the patient and family/significant other in meeting nursing and related needs.

Participate in and present in-service programs
-
Participate in peer review and Quality Assessment and Performance Improvement as assigned.

Understand and adhere to established Agency policies and procedures.
-
Process orders and notify physicians of patient needs and changes in condition.

Completes certification and recertification orders and discharge summaries
-
Complete and submit OASIS Assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with agency-defined time frames.

Determines the amount and type of nursing intervention needed by each patient

Give total patient care as needed

Appropriately utilize ICD-10 codes.

Takes On-Call duty nights, weekends, and holidays as assigned

Supervises and teaches other nursing personnel
Work Environment: Work onsite in Agency’s Office; Travel to/from patient homes; work in patient homes
