Compensation: $105,000 - $125,000 Annual Salary, plus RVU bonuses
Job Type: Full-Time, in-person

Job Description: Our Skilled Nursing Facility in Rochester, NH is looking for a Nurse Practitioner to join a facility-based team of medical providers caring for patients living in long-term care facilities.

Job Qualifications:
- Current, unrestricted NP license in the state of New Hampshire
- Board certification
- DEA licensure
- Clinical background in adult, family or geriatric specialties
- Experience working in a long-term care setting
Responsibilities include but are not limited to:
- Provide primary and preventative care for long-term care patients residing in skilled nursing facilities
- Manage residents with an acute change in condition
- Advanced care planning and chronic care management
- Order, review and follow up on treatments and diagnostic testing
- Communicate regularly with nursing home staff and collaborate with the primary care physician
