Location: Worcester, MA
Setting: Long-Term Care / Skilled Nursing Facility
Schedule: Night Shift
Pay: $36–$39/hr
Job Type: Full-time - Permanent Direct Hire
Summary:
Provide quality patient care in a long-term care setting, including medication administration, patient monitoring, documentation, and coordination with the healthcare team.
Responsibilities:
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes to physicians/RNs
- Maintain accurate patient records and documentation
- Assist with admissions, discharges, and daily patient care
- Support and communicate with residents and family members
Requirements:
- Active Massachusetts LPN license
- CPR/BLS certification preferred
- Long-term care or rehab experience is a plus
- Ability to work in a fast-paced team environment
- Strong communication and organizational skills
