Facilities include: Cambridge Rehabilitation and Nursing Center 8 Dana Street CambridgeSancta Maria Nursing Facility 799 Concord Avenue CambridgeLighthouse Nursing Care Center 204 Proctor Avenue RevereRehabilitation and Nursing Center at Everett 289 Elm Street Everett Plunkett floorStart Date: 6/29/26Assignment Length: Three to six monthsPatient Population:Short term rehabilitation and long term care patientsEverett location is long term care onlySchedule: Monday through Friday 9am to 5pmNo call requiredPatient Volume: Six to eight patients per day including new admissions and follow up visitsCare Model:On site Nurse Practitioner support at each facilityNPs manage nursing calls orders and medication refillsNPs complete initial admission visit within seventy two hoursPhysician completes new admission visit typically within seven daysLong term care patients require physician visits every sixty to ninety days unless acute needs ariseEMR: Epic and PointClickCare. Epic training and coding training required
