Licensed Practical Nurse – Long-Term Care (LTC)
Location: Laconia NH
Rate: Up to $1,450.00 Gross Weekly + Benefits Available from Day 1
Specialty: Long-Term Care (LTC)
License Required: Active, Unencumbered New Hampshire (NH) LPN License
Shifts Available: 8-Hour Evenings
Contract: 13-Week Contract | Local & Travel Accepted
Requirements
- Minimum 2 years of Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience
- Active, unencumbered New Hampshire LPN license
- Previous travel nursing experience preferred
- Experience caring for geriatric and medically complex residents
- Electronic Medical Record (EMR) documentation experience required
- Charge Nurse experience highly preferred
- Must be able to float as needed
Certifications
- AHA BLS required
- Additional certifications as required by the facility
Responsibilities
- Provide comprehensive nursing care to residents in a long-term care or skilled nursing facility
- Assess, monitor, and document residents' health status and changes in condition
- Administer medications and treatments in accordance with physician orders and facility protocols
- Develop, implement, and evaluate individualized resident care plans
- Monitor chronic conditions and promote residents' health, safety, comfort, and overall well-being
- Collaborate with physicians, nursing assistants, therapists, residents, and families to ensure quality care
- Maintain accurate and timely documentation in the facility's electronic medical record system
- Follow infection prevention protocols, medication safety standards, and applicable state and federal regulations
- Recognize changes in residents' conditions and promptly communicate concerns to the appropriate healthcare providers
- Float as needed to support resident care and facility operations
