A Clinical Liaison serves as the key connection between patients, healthcare providers, referral sources, and the organization. The role focuses on building strong relationships with hospitals, physicians, case managers, and other healthcare professionals while coordinating patient referrals and ensuring appropriate access to services.
Key Responsibilities:

Develop and maintain relationships with hospitals, physicians, case managers, and referral partners.

Identify and evaluate potential patient referrals based on clinical and organizational criteria.

Educate patients, families, and healthcare professionals about available services.

Coordinate admissions, referrals, and transitions of care.

Conduct patient assessments and gather relevant clinical information.

Communicate effectively with clinical, administrative, and referral teams.

Track referrals, admissions, and outreach activities.

Represent the organization at healthcare events and community meetings.

Maintain accurate documentation and comply with HIPAA and other applicable regulations.

Support business development and community outreach initiatives.
Qualifications:

Bachelor’s degree in nursing, healthcare administration, social work, marketing, or a related field preferred.

Clinical or healthcare experience strongly preferred.

Excellent communication, interpersonal, and relationship-building skills.

Knowledge of healthcare referral and discharge processes.

Ability to work independently and collaborate with multidisciplinary teams.

Strong organizational and time-management skills.
