Registered Nurse (RN) - Case Manager
Location: Worcester County, MA
Job Type: Full-Time | Permanent | Direct Hire
Schedule: Days | 40-hour salaried position | 8:00 AM - 5:00 PM
Compensation
$85,000 - $97,000 annually
Join an established hospice organization with approximately 20 years of experience and a strong, long-tenured team. Hospice/Home Health experience is preferred, but experienced LTC RNs will also be considered. This role focuses on compassionate, patient-centered end-of-life care using a whole-person mind, body, and spirit approach.
Responsibilities:
- Complete comprehensive patient and family assessments
- Develop and revise individualized plans of care
- Provide skilled nursing care and symptom management
- Coordinate care with physicians, facilities, families, and the interdisciplinary team
- Maintain timely and accurate EMR documentation
- Order medications, DME, and supplies
- Supervise ancillary staff and coordinate visits
- Participate in on-call rotation and IDG meetings
Requirements:
- Current RN license
- Hospice/Home Health experience preferred
- LTC experience considered
- Strong assessment and critical thinking skills
- Excellent communication and organizational abilities
- Ability to work independently and collaboratively
- Compassionate approach to end-of-life care
