RN Case Manager
\n Will look at RNs with LTC experience! Hospice/Home Health experience preferred.

\n - Worcester County

\n 40-hour salaried full-time positions with participation in on-call coverage as required.
$85,000-$97,000 annually.
\n Save this info and don't use it right away: Offering a $5,000 sign-on bonus, paid out quarterly over the course of one year. ($1k, $1k, $1k, $2k)
18% fee with 45-day guarantee

\n Census throughout the state of Mass is close to 100.
\n Been around for 20 years and used to be family-owned before partnering with Echo.
\n 8a-5p business hours. Once a month on call rotation as far as call frequency.
\n 10-15 patients Case Managed for an RN
\n 5-6 visits a day typically
\n Admission and recert visits may mean being flexible in readjusting your schedule

\n Large longevity of staff. 70 employees total
\n Good director and clinical manager in place - Jamie and Vanessa

\n Key responsibilities include:
\n - Completing comprehensive patient and family assessments
\n - Developing and revising individualized plans of care
\n - Providing skilled nursing care and symptom management
\n - Coordinating care with physicians, facilities, families, and the interdisciplinary team
\n - Managing documentation timely and accurately within the EMR system
\n - Ordering medications, DME, and supplies as appropriate
\n - Supervising ancillary staff and coordinating visit schedules
\n - Participating in on-call rotation and IDG meetings

\n The ideal candidate will have:
\n - Current RN licensure
\n - Hospice and/or home health experience preferred
\n - Strong assessment and critical thinking skills
\n - Excellent organizational and communication abilities
\n - Ability to work independently while collaborating with the team
