Job DescriptionRN – Home Health
Location: South Dennis, MA
Duration: 13 Weeks
Schedule: 40 Hours/Week | Monday–Friday | Occasional Weekends & Holidays
Responsibilities
- Perform comprehensive nursing assessments upon patient admission, including physiological, psychosocial, environmental, and health-history factors.
- Integrate assessment findings to identify patient problems and develop an appropriate plan of care.
- Utilize HHRG values to establish visit frequency and appropriate interdisciplinary resource utilization.
- Evaluate clinical needs and initiate, develop, implement, and revise individualized plans of care.
- Participate in interdisciplinary patient case conferences and evaluate the effectiveness of care plans.
- Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.
- Notify physicians and other involved care providers of significant changes in patient condition.
- Complete admissions and required paperwork accurately and on time.
- Document skilled nursing care clearly and concisely, including patient/caregiver teaching, direct care, and responses.
- Obtain and clarify valid physician orders for all care provided.
- Plan and coordinate patient discharges and communicate discharge information to patients and physicians.
- Complete all required discharge documentation.
- Refer patients to appropriate community resources.
- Perform other duties as assigned.
Requirements
- Current, unencumbered Massachusetts RN license and other credentials required for the specialty.
- Successful completion of the VNA pre-hire medication test.
- Minimum 1 year of nursing experience, or 6 months of nursing experience with a BSN.
- CPR certification required.
- Home health and IV experience preferred.
- Computer skills preferred.
- BSN preferred.
