- Manage an assigned caseload and coordinate patient care across the full episode of illness
- Collaborate with interdisciplinary teams to support quality, clinical, and financial outcomes
- Assess patient needs using care guidelines, clinical protocols, and payer requirements
- Coordinate resources and services from pre-admission through post-discharge
- Support efficient care delivery while improving quality, cost, and patient satisfaction
- Monitor patient response to therapies and adjust care coordination as needed
