- Must have OASIS (Outcome and Assessment Information Set) Experience
- Home Health and Hospice Experience required
- Experience traveling from patient to patient for home visits
- Must be able to manage and adjust to changes in schedule
- Must have drivers license in good standing
- BLS required
- EMR system used is HealthWyse (Careficient)
- Outpatient care
- Facility does their own Background Check as well
