Responsibilities:

- Provide direct nursing care to hospice patients in their homes.
- Make home visits under the supervision of an RN Case Manager.
- Assess and monitor patients' physical and emotional needs.
- Perform nursing interventions according to the hospice care plan.
- Educate patients and families regarding hospice and end-of-life care.
- Communicate changes in patient condition to the RN and care team.
- Assist the RN and medical provider with specialized end-of-life care.
- Document and report patient responses and changes in condition.
- Collaborate with the interdisciplinary hospice team.

Requirements:
\n
- \n
- Active, unencumbered LPN license in Vermont and New Hampshire OR compact LPN license.
- Graduate of an approved practical nursing program.
- Experience in medical/surgical nursing and/or home health care.
- Hospice experience is beneficial.
- Ability to work independently and make home visits.
- Strong communication and patient/family education skills.
