Description\n

American Traveler is seeking a Home Care Registered Nurse for a 13-week assignment requiring OASIS experience and a VT or compact RN license.

  • Home health/hospice setting providing in-home nursing care to assigned patients
  • \n

  • Serves as Case Manager, managing caseloads and visit times per agency productivity standards
  • \n

  • Schedule options include 8am-4pm Tuesday-Saturday with some on-call, or Monday-Friday with weekends and some on-call
  • \n

  • Holiday coverage required
  • \n

  • Utilizes McKesson for documentation and visit processes
  • \n

Requirements\n

    \n
  • Active VT or compact RN license required upon acceptance
  • \n

  • Minimum 2 years of nursing experience required
  • \n

  • OASIS documentation experience required
  • \n

  • Knowledge of high-tech nursing skills including IV therapy, wound VAC, and complex wound care
  • \n

  • Ability to complete Agency, State, and Federal forms such as HHABN and Medicare Notice of Non-Coverage
  • \n

Additional Information\n

  • Conducts patient/caregiver assessments and develops individualized plans of care in accordance with evidence-based practice and state/federal regulations
  • \n

  • Collaborates with LPNs, physicians, and interdisciplinary team members to coordinate patient care
  • \n

  • Provides patient and caregiver education on medications, treatment options, and discharge instructions
  • \n

  • Coordinates safe patient discharges and transfers, ensuring timely completion of required paperwork
  • \n

  • Participates in monthly peer review audits and case conferences with team members
  • \n

  • Reports patient incidents to the QI Specialist in a timely manner
  • \n
