About the Role:

As a Registered Nurse (RN) at our Weathersfield, Vermont facility, you will be responsible for providing high-quality, patient-centered care to our residents. Your expertise will be invaluable in creating individualized care plans, implementing medical treatments, and collaborating with our interdisciplinary team to ensure that our residents receive the best possible care.

Responsibilities:

• Provide direct patient care to a diverse population of residents, including administering medications, performing treatments, and documenting patient information
• Develop and implement individualized care plans, ensuring that each resident receives appropriate diagnostic, therapeutic, and preventative services
• Collaborate with the interdisciplinary team, including physicians, therapists, and other healthcare professionals, to ensure comprehensive care
• Monitor and report resident conditions, including identifying and addressing any signs of decline or improvement
• Participate in quality improvement initiatives to enhance patient care and outcomes
