This position is ideal for someone who is passionate about providing personalized nursing care to residents with diverse levels of independence, ranging from assisted living to skilled nursing.

Key Responsibilities:

• Conduct thorough assessments of residents' health and needs.
• Administer medications as prescribed.
• Provide wound care and manage treatment plans.
• Develop and implement individualized care plans.
• Maintain accurate documentation of care provided.
• Collaborate effectively with an interdisciplinary team to ensure comprehensive care.

Qualifications and Requirements:

• Current RN or LPN license.
• Experience in a nursing role, preferably in a long-term care setting.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a team-oriented environment.
