Advanced Practice Provider (Nurse Practitioner / Physician Associate) — Pain Management
Compensation 170k to 200k (base + bonus)

Locations: MA, NH, RI, ME (multi-state expansion)
BHMG Pain Management is a rapidly growing, physician-led medical group specializing in post-acute and skilled nursing facility care. We provide evidence-based, patient-centered pain management to medically complex patients in rehabilitation and long-term care settings.

Position Overview
We are seeking a full-time Advanced Practice Provider (NP or PA) to deliver specialized pain management care across partnered skilled nursing facilities.
Pain Management training will be provided.
This is a field-based role ideal for clinicians who want:
- Autonomy with structured support
- Predictable daytime schedule (no overnight call required)
- High clinical impact in a rapidly growing service line
Key Responsibilities
- Perform initial pain evaluations and ongoing management visits in SNF/post-acute settings
- Develop and implement multimodal pain management plans (non-opioid, opioid stewardship, adjuvant therapies)
- Perform medication optimization, taper plans, and risk mitigation strategies
- Collaborate with:
- Facility staff (nursing, rehab, administration)
- Primary care providers
- Psychiatry and behavioral health teams (when appropriate)
- Document encounters in a timely manner using compliant, audit-ready templates
- Participate in care coordination and provide education to facility staff on pain management best practices
- Escalate complex cases per protocol
Clinical Model
- Moderate visit frequency model focused on appropriate follow-up cadence
- Emphasis on:
- Functional outcomes
- Safe prescribing
- Regulatory compliance (Medicare, MA plans, SNF regulatory environment)
- Structured clinical workflows and documentation support
Schedule
- Full-time (Monday–Friday)
- Daytime hours
- No overnight call
- Optional flexibility based on territory
Qualifications
Required:
- Licensed Nurse Practitioner (NP) or Physician Associate (PA)
- Minimal 2 years of practice in any specialty
- Active and unrestricted license in applicable state(s)
- DEA license (or eligibility)
Preferred:
- Experience in geriatrics, internal medicine, SNF, or post-acute care
- Experience in pain management or palliative care
- Familiarity with polypharmacy and complex chronic disease management
- Strong documentation and compliance awareness
