Job Summary:\n

We are seeking a compassionate and experienced Registered Nurse (RN) – Hospice to join a dedicated home hospice team providing high-quality, patient-centered end-of-life care. This position offers the opportunity to deliver one-on-one hospice and palliative care to patients in the comfort of their homes while collaborating with an interdisciplinary care team to ensure compassionate, individualized support for patients and their families.

The ideal candidate will have strong home health and hospice experience, be proficient with OASIS documentation, and possess the ability to independently manage patient caseloads.

Responsibilities\n
- \n
- Provide comprehensive hospice and palliative nursing care in patients' homes.
- Perform patient assessments and develop individualized plans of care.
- Deliver expert pain and symptom management to promote comfort and quality of life.
- Complete OASIS assessments and maintain accurate, timely clinical documentation.
- Manage an assigned patient caseload while coordinating ongoing care.
- Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and end-of-life care.
- Collaborate with physicians, hospice interdisciplinary team members, and community healthcare providers to ensure continuity of care.
- Respond to changing patient needs with compassion, professionalism, and clinical expertise.

Required Qualifications\n

- Active Registered Nurse (RN) license
- Current BLS Certification
- Recent Home Health experience (Required)
- Recent Hospice nursing experience (Required)
- OASIS documentation experience (Required)
- Previous case management experience in a home care setting preferred
- Strong clinical assessment, communication, and patient education skills
