Job Title: Registered Nurse – Hospice Home Health
Location: Lawrence, MA
Rate: Up to $2,200 Gross Weekly + benefits available from day 1
Shift: Days, 8:00 AM – 5:00 PM, Monday–Friday
Contract: 13 weeks
Hours: 40 hours/week
Requirements:
<ul>
<li>Minimum 1 year of recent hospice experience</li>
<li>Recent hospice experience required; first-time travelers not accepted</li>
<li>Active, unencumbered Massachusetts RN license</li>
<li>Reliable transportation for home visits</li>
<li>Current driver's license</li>
<li>Ability to travel between patient homes</li>
</ul>
Certifications:
<ul>
<li>AHA BLS</li>
</ul>
Responsibilities:
<ul>
<li>Provide skilled hospice nursing care in patients' homes according to physician orders and care plans</li>
<li>Assess patients and provide appropriate nursing interventions</li>
<li>Complete hospice visits, including Start of Care and follow-up visits</li>
<li>Monitor patient conditions and communicate changes to the appropriate care team</li>
<li>Provide education and support to patients and their families</li>
<li>Complete accurate and timely clinical documentation</li>
<li>Collaborate with physicians and the interdisciplinary care team</li>
