RN Hospice Clinical Coordinator, full time, M-F, 7:30-4:00, Needham/Woburn areas

Job Summary:
We are seeking a compassionate and skilled Hospice Clinical Coordinator to join our team on a full-time basis. The Hospice Clinical Coordinator will be responsible for overseeing and coordinating all clinical activities within our hospice program to ensure high-quality care for patients.

Compensation based on experience $89,440 - $110,240 per year

Qualifications:

  • Bachelor's degree in Nursing or related field required; Master's degree preferred
  • Active Massachusetts RN license
  • Minimum of 3 years of experience in hospice care
  • Strong knowledge of hospice regulations and guidelines
  • Excellent communication and interpersonal skills
  • Ability to lead and mentor clinical staff
  • Detail-oriented and organized

Functions & Responsibilities:

  • Triages and responds to calls from patients, families, referral sources, and team members.
  • Responds to patient's needs for direct care by planning and coordinating the care with appropriate clinical staff and the hospice scheduler.
  • Obtains and assesses clinical information regarding patient's condition and responds accordingly and timely using accepted standards of nursing practice.
  • Supervises processes and practices at the pod level to ensure clients receive the appropriate level of care in conjunction with the Clinical Manager.
  • Coordinates referrals for Hospice from hospitals, physicians, other health facilities, community agencies, clients/families and VNA Care.
  • Communicates with the client's physician, family, appropriate staff and others to obtain complete referral information, share referral information and to assist in organizing resources necessary for client care prior to admission.
  • Communicates with referral sources, families and physician to confirm the plan for Hospice admission. Works closely with liaison staff.
  • Accepts telephone orders from physicians, and routes information to appropriate caregiver and team including notifying staff in the field as necessary.
  • Demonstrates initiative and takes action to satisfy clients and takes responsibility for ensuring a positive experience for outcomes.
  • Maintains current knowledge of Hospice and Medicare/Medicaid regulations as well as Hospice licensure requirements.
