MDS Coordinator - Nurse RN or LPN
Job Summary:

Responsibilities:

Conduct and coordinate the development and completion of the resident assessment

Conduct resident assessments in accordance with current rules, regulations, and guidelines

Implement resident CAA's

Participate in facility survey readiness and throughout the survey

Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident
Requirements:

Current unencumbered licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Vermont

Two years of MDS and PPS experience

Care Planning experience
