**Join Our Team as a Home Care Registered Nurse!** **Job Description:** - Utilize the nursing process to assess patient and caregiver needs, ensuring quality care for assigned patients. - Develop and communicate a tailored plan of care for each patient, adhering to evidence-based practices and regulations. - Serve as the Case Manager for assigned patients, actively managing caseloads and visit schedules. - Complete OASIS documentation accurately and timely, keeping updated with OASIS requirements. - Implement medical plans of care and nursing interventions to safeguard and enhance patient care quality. - Stay up-to-date on advanced skills, including IV maintenance and complex wound care, ensuring infection control standards are upheld. - Collaborate with LPNs within their scope of practice, ensuring care plans are executed effectively. - Evaluate the effectiveness of care plans by assessing patient responses and adapting strategies as necessary. - Maintain accurate documentation to support ongoing quality care integration among healthcare teams. - Follow agency protocols with McKesson processes, ensuring efficient visit management. - Educate patients and caregivers on medication effects, treatment options, and care instructions, promoting informed decision-making. - Identify interdisciplinary needs for patients and communicate essential information promptly to other disciplines. - Complete necessary agency and regulatory forms accurately and promptly ensuring availability during visits. - Conduct timely visits without compromising care quality or coordination. - Assess patient needs for HCA services and develop a comprehensive plan of care with ongoing supervision. - Transmit patient incident reports to Quality Improvement specialists promptly as required. - Coordinate safe patient discharges and transfers, handling paperwork and patient information meticulously. - Collaborate with physicians, providers, and team members to ensure the best possible patient care. - Participate in monthly peer review audits, providing and receiving constructive feedback professionally. - Schedule and facilitate case conferences proactively, enhancing team communication and documentation. - Advocate for patient care issues, ensuring services are tailored to meet each patient’s unique needs. **Shift Information:** - 8 am to 4 pm, Tuesday - Saturday or Sunday - Thursday - On-call and holiday work is required **Minimum Years of Experience:** - 2 years **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
