Schedule: Monday-Friday, 4 or 5 weekdays
Setting: Skilled Nursing Facilities, Assisted Living Facilities, Memory Care, and Residential Settings
Job Overview:
We are seeking an experienced Nurse Practitioner or Physician Assistant to provide primary and acute care services to patients in Skilled Nursing Facilities and Assisted Living Facilities throughout Cape Cod, MA and surrounding areas. The ideal candidate will have previous acute care experience as an NP or PA, along with experience caring for patients in post-acute, long-term care, assisted living, or primary care settings.
The provider will deliver comprehensive patient care, including primary care visits, medication reconciliation, annual wellness visits, chronic disease management, diagnosis and treatment, and patient/family education.
Key Responsibilities:

Perform comprehensive examinations and review patient medical records

Provide primary and acute care services to patients in skilled nursing and assisted living settings

Conduct medication reconciliations and medication management
-
Perform annual wellness visits and routine follow-up evaluations

Order and interpret diagnostic tests, including laboratory tests and imaging

Diagnose and treat acute and chronic medical conditions

Develop and implement appropriate treatment plans

Prescribe medications and monitor treatment effectiveness

Manage chronic and infectious diseases

Maintain accurate and timely clinical documentation

Educate patients and families regarding diagnoses, medications, treatment plans, and preventive care

Coordinate care with physicians, nurses, facility staff, patients, and families

Monitor patients' overall health and promote preventive healthcare practices
-
Maintain professional knowledge and stay current with clinical best practices
Requirements:
-
Active Nurse Practitioner or Physician Assistant license
-
Previous experience as an NP or PA in acute care
-
Experience in skilled nursing facilities, assisted living, primary care, or similar healthcare settings

Strong clinical assessment, diagnostic, and treatment skills

Ability to manage acute and chronic medical conditions

Experience with medication reconciliation and medication management

Strong communication and interpersonal skills

Ability to work independently while collaborating with an interdisciplinary care team
