Position Overview
Speech &amp; Language Pathologists provide services to develop, enhance, or restore functional communication, speech, language, and/or feeding/swallowing skills of clients whose abilities to communicate effectively and/or to eat safely and efficiently are threatened or impaired by physical, cognitive, social-emotional, and sensory impairments.
 
Speech &amp; Language Pathologists at NAPA Center complete skilled assessments and provide skilled intervention to infants, toddlers, children, and adolescents with a variety of diagnoses that impact speech production, receptive language, expressive language, social-pragmatic skills, and feeding/swallowing skills within the professional scope of practice. The role requires all therapists to work as part of a multi-disciplinary team and provide evidence-based interventions to clients that include complex communication and feeding needs.
 
Requirements
<ul>
<li>Minimum qualifications include completion of at least a Master of Arts or Applied Science in Speech-Language Pathology, Communication Disorders, Communication Sciences and Disorders, or equivalent, from an accredited university.</li>
<li>Must have and maintain state license with the appropriate licensing board.</li>
<li>Must have and maintain ASHA CCC credentials.</li>
<li>Strong clinical experience - at least 2 years post graduate experience, preferably in a pediatric setting with exposure to children and/or young adults with complex disabilities.</li>

<li>Maintain impeccable documentation. Therapists have a dedicated documentation hour built into their schedule each day and are expected to use this time for report writing, including but not limited to evaluations, progress reports, school district annual IEP reports, assistive technology. Therapists are expected toensure that treatment notes are recorded accurately and timely in the electronic health record system, according to the policies of NAPA Center.</li>

We are seeking a Youth Navigator as part of CTI’s Youth Services team located at the Youth Opportunity Center.
The Youth Navigator will be responsible for case management for homeless and at risk youth and young adults between the ages 16-24. The Youth Navigator provides advocacy, support, and community connection to assist participants and families to overcome barriers to self-sufficiency and stability, while assisting young adults in accessing support from across the Greater Lowell Area.
Responsibilities Include:

<li>Supporting participants in accessing basic needs</li>
<li>Assessing individual stability</li>
<li>Hold regularly scheduled case management sessions with participants</li>
<li>Formulation of youth lead - Individualized Service Plans</li>
<li>Connect with community partners and resources</li>
<li>Assisting other Youth Services team members as needed</li>
<li>Supporting programming and difficult or complex situations</li>
<li>Meeting with young adults in the community to allow for better access to services</li>

Qualifications:
<ul>
<li>Bachelor’s Degree preferred, preferably in a Human Services related field</li>
<li>5+ years’ direct case management experience OR 2+ years of experience working with at-risk youth may substitute for degree</li>
<li>Ability to work comfortably with at-risk youth in both group and individual settings, preferably with experience supporting at risk youth entering into employment and post-secondary education</li>
<li>Minimum of one year assessment, advocacy and case management work in a Human Service field required</li>
<li>Demonstrated knowledge of community resources available to low-income individuals and families</li>
<li>Excellent customer service, organizational, oral and written communication skills</li>
<li>Ability to manage multiple priorities and work in a fast-paced environment</li>
<li>Proficient computer skills required in basic computer software and data collection systems</li>

Job Purpose:
The Nurse will function as a dynamic member of the patient care team, providing education and guidance to fertility patients throughout their journey. The Registered Nurse will serve as treatment coordinator and strive to provide every individual an optimal patient experience. 
Essential Functions and Accountabilities:

<li>Implements Provider directives through management of diagnostic and clinical treatment plans.</li>
